Accessing Your Health Records
You have the right to request and receive copies of your health care records in an electronic format.

How to Request: Please submit a written request via the Simple Practice Client Portal, or by emailing kinga@cc-therapy.org.

Timeline: In compliance with Texas law, your records will be provided within 15 business days of receipt of your written request.
