Accessing Your Health Records

You have the right to request and receive copies of your health care records in an electronic format.

  • How to Request: Please submit a written request via the Simple Practice Client Portal, or by emailing kinga@cc-therapy.org.

  • Timeline: In compliance with Texas law, your records will be provided within 15 business days of receipt of your written request.

 

Licensing & Disciplinary Authorities

This practice is regulated by the professional boards in the states where services are licensed. You may contact these authorities to verify a license or discuss professional conduct:

  • Texas: Texas Behavioral Health Executive Council (BHEC).

  • Illinois: Illinois Department of Financial and Professional Regulation (IDFPR).

 

Filing a Consumer Complaint

If you have a complaint regarding data privacy, business transparency, or consumer protection violations in the state of Texas, you may file a formal complaint with the Office of the Attorney General:
